A PIA Pakistan International Airlines Avions de Transport Regional ATR-72-212A, registration AP-BKX performing flight PK-580 from Karachi to Multan (Pakistan), was accelerating for takeoff from one of the runways 25, when the crew rejected takeoff due to a problem with the right hand engine (PW127) and returned to the apron.

Pakistan's AIB rated the occurrence a serious incident and opened an investigation.

Pakistan's BASIP released their final report on Aug 6th 2026 concluding the probable causes of the serious incident were:

Cause

The aborted take-off occurred as a result of a fracture of the second-stage power turbine blade(s) in the PW127M engine. The fracture originated from leading-edge cracks caused by Type I sulphidation hot corrosion, which propagated by fatigue and culminated in overload failure of the blade(s).

Configuration

The blade configuration installed on the incident engine remained susceptible to sulphidation-related leading-edge degradation despite incorporation of SB 21876.

Known in-service trend

Prior to the incident, P&W Canada had identified a continuing trend of secondstage power turbine blade distress involving post-SB 21876 blades and had issued SB 21917 and SIL PW100-188 as further mitigation measures.
